Work Permits

Work permits are required for minors under the age of 16 who wish to work in Illinois.

Permits are issued to Glenbrook North High School students under the age of 16 and to minors who reside within the high school's community.

Before applying for a permit, the minor needs to find a job and obtain a "statement to employ" from the prospective employer. In accordance with the State of Illinois guidelines, the application for work permit shall be submitted in person by the minor desiring employment. The minor shall be accompanied by his/her parent, guardian, or custodian. The following materials need to be submitted to the issuing officer:

  • Employer's Statement of Intention to Employ Minor (print form) or a statement of intention to employ on letterhead stationery which includes: description of specific occupation or duties, number of hours per day the student will work, and number of days per week the student will be employed
  • Application for Illinois Work Permit (print form)
  • Minor Social Sercurity number
  • Statement of physical fitness to work or copy of a current physical (issued within the last twelve months)
  • Evidence of age (one of the following: birth certificate, passport, baptismal certificate, or any other document to prove date of birth)
